Create a patient account on our website
Complete all information about your child
Add any other children to your profile via family members on the left side panel
Schedule a New Patient Establish Care Visit for each child. Once this visit is completed, tell the provider you want to activate a monthly membership. (Please note, at this time, you cannot enroll into the membership, only the practice can do so)
You will be able to schedule appointments through your portal, view documents, and much more!
